Home Health Care Skilled Providers and Available Services

The experienced staff of Lake Forest Hospital Home Health Care offers a wide variety of services that range from personal care to high-tech infusion services.  Our team is a caring, diverse group of professionals committed to providing the highest-quality home care and comfort.

SKILLED NURSING
A registered nurse will develop a care plan under the direction of your physician along with you and your family to assure that all aspects of medical care that you need are provided.  The home health services of a registered nurse can include:

  • Bowel and bladder training
  • Catheter care
  • Diabetic care and education
  • Infusion of antibiotics, hydration, TPN
  • Medication management
  • NG/gastrostomy tube care
  • Ostomy care and education
  • Post-operative care and education
  • Skilled assessment, teaching/instruction
  • Teaching enteral feeding and nutritional care
  • Tracheostomy care and education
  • Wound care education for caregivers

 

PHYSICAL THERAPY
A licensed physical therapist will provide an in-home assessment, evaluating your level of functioning and then work with your physician to develop a home exercise program that will help you restore functioning to the highest level.  A physical therapist's services can include:

  • Evaluation of equipment needs and teaching safe use of equipment
  • Evaluation of gait, ambulation and home safety
  • Mobility, gait, transfer and prosthetic training
  • Post-operative rehabilitation and education
  • Therapeutic exercise
  • Weight-bearing exercise
  • Written home exercises

 

OCCUPATIONAL THERAPY
A licensed occupational therapist will provide an in-home evaluation of your activities of daily living (ADLs).  Then the therapist works with your physician to develop activities that will help restore your ability to perform ADLs.  The occupational therapist can:

  • Adapt tools used in daily living (eating utensils, dressing devices)
  • Provide exercises that will promote ADL retraining
  • Provide fine-motor skill training
  • Provide neuro-developmental sensory treatment
  • Provide perceptual motor training

 

SPEECH THERAPY
A licensed speech therapist will evaluate your speech pattern and swallowing ability.  The speech therapist will work with your physician to develop a therapy plan that will improve your speech function to the highest level.  The services of a speech therapist can include:

  • Communication of your ongoing needs
  • Development of an in-home oral program that will optimize the level of oral-motor
  • Dysarthria — slurred speech therapy
  • Dysphagia — swallowing therapy
  • Evaluation of speech pattern and swallowing ability
  • Post CVA rehabilitation
  • Treatment of cognitive disorders
  • Treatment of receptive aphasia

 

HOME HEALTH AIDES
Under the direction and supervision of a registered nurse or therapist, a home health aide can provide and assist you with personal care and hygiene.  They also help you maintain a safe environment.  The home health aide's srevices can include:

  • Assistance with bathing, oral hygiene
  • Assistance with household services essential to health care
  • Assistance with simple dressing changes
  • Assistance with transferring, walking and exercise to promote independence
  • Complete or partial bed bath to promote independence
  • Medication reminders (when set up by family or RN)
  • Personal care assistance to promote independence

 

MEDICAL SOCIAL WORKERS
Along with skilled registered nurses or therapy services, a medical social worker will work with the home health care team and can provide:

  • Assistance with advance directives
  • Counseling
  • Evaluation of emotional state
  • Long-term care planning
  • Resource planning

Additional Services

INFUSION SERVICES
Our trained nurses can administer a wide variety of infusion therapies in the comfort of your home.  These services can include:

  • Factor products
  • Hydration
  • IV antibiotics
  • Pain management
  • PICC line care
  • Total parenteral nutrition

Our home infusion pharmacy is operated by a national provider of high-tech home care services accredited by the Joint Commission.

 

HOME MEDICAL EQUIPMENT
The importance of proper equipment for patient care cannot be underestimated.  We have a full line of medical, convenience and safety equipment available for the home.  And our team of certified and registered respiratory therapists is equipped to provide home respiratory services and therapeutic procedures including:

  • Aerosol therapy
  • BIPAP
  • Enteral nutrition
  • Nasal CPAP
  • Oxygen
