Skilled care is provided by RNs and therapists with diverse clinical backgrounds in the care of patients with acute and chronic illness.
These services, when ordered and managed by your physician, instill confidence as you gradually gain independence. We are dedicated to helping you improve your quality of life after leaving the hospital or any other time you require assessment, education and skilled intermittent care.
Scope of Professional Services
Our team is a caring, diverse group of professionals committed to providing the highest-quality home care and comfort. Registered nurses; physical, occupational and speech therapists; medical social workers; and home health aides work with you and your family to promote an optimal level of healthy independence.
We also have a certified wound care nurse who consults with staff and patients regarding complex wounds that are hard to heal. Additional services can be provided by our dietitian, diabetic specialist and chaplain.

What is Home Health Care?
The criteria that make a beneficiary eligible for home health care is defined by Medicare and most insurances as "skilled care, ordered by a physician, to a homebound individual, intermittently and for a short duration, after acute illness or an exacerbation of chronic illness, in a safe environment."
An individual is considered homebound if leaving home requires taxing effort, the help of another individual or the use of an assistive device. It is usually due to illness or injury that an individual's ability to leave home is restricted. In general, time away from home should be of short duration and infrequent.
