We are dedicated to providing exceptional service and accurate and complete communication is what sets us apart.
On-Site or Off-Site—Our occupational health service professionals stand ready to respond with prompt, appropriate care. With dedicated occupational health physicians in both our Emergency Department and our full-service locations in Lake Forest and Gurnee, we offer convenient, 24-hour personalized service.
Acute Injury Care—Immediate treatment of an injured employee is the first step toward the best possible outcome with the least time lost from work. At Lake Forest Hospital’s Occupational Health Program, we pride ourselves on prompt, appropriate care. Our physicians are trained to treat work-related injuries. They respond to light duty availability and coordinate care of the injury with the employee and employer.
Exceptional Service/Integrated Case Management—Our health service professionals are experts in coordinated care, from timely reporting outcomes of drug/alcohol tests to making sure you are informed during every step of your employee’s return-to-work process.
Before your employee leaves our clinic, our nurse makes appointments for follow-up care, including Physical/Occupational Therapy close to home or work and specialist referrals, if needed. Appointments are typically available within two working days. We realize the necessity of scheduling follow-up care outside working hours and make every attempt to accommodate these requests. Employers are informed of the scheduled appointment.
Wellness Programs—Preventing illness and injury begins by teaching people how to live safe and healthy lives. Lake Forest Hospital Occupational Health Program, provides consultations and educational programs for your employees at your work site. Our professional staff of exercise specialists, respiratory, physical and occupational therapists, registered dieticians and certified health educators bring a wealth of information and expertise to your door.
Executive Physical Program—Companies have found that executive physicals are valued by employees and are cost effective for the organization. By encouraging healthier lifestyles and identifying previously undetected health problems, executive physicals can result in improved health and wellness, reduced disability, lower medical costs, and improved job performance.
Our executive physical program participants receive a comprehensive physical examination and health risk appraisal individualized by gender, age, and lifestyle and coordinated by one of our board-certified Lake Forest Hospital physicians. Following the visit, participants will receive a summary that provides a profile of current health status, identifies current and potential health risks and gives recommendations to promote wellness and prevent illness.
International Travel Program—As increasing numbers of people travel internationally, our employer clients have requested an international travel program. Our carefully coordinated program provides employers with health history questionnaires that are completed by the traveler and returned to us with their travel schedule. Then, our physician and nurse develop a travel plan that includes required and recommended vaccinations specific to those areas of travel. We supply educational materials, necessary prescriptions, and letters pertaining to medical health required upon entering a country.
Healthy Work Site/Return to Work Services—Healthy work site and return to work services are available at the work site or through Lake Forest Hospital’s industrial rehabilitation program. Our return to work program offers highly structured, goal-oriented, individual treatment designed to maximize an individual’s ability to continue to work or return to work.
Services include ADA consultative services, education and safety programs, ergonomic assessments, functional capacity assessments, job task analysis, job/tool modification, on-site work program, work hardening (fitness & conditioning) and work reconditioning.
Center for Pain Treatment & Rehabilitation
The Center offers an experienced team, including anesthesiologists who are board certified by the American Board of Anesthesiology, who provide a complete range of pain rehabilitation and pain management services. We can help you deal with the physical, emotional, social and vocational effects of your chronic pain condition so that you can resume your activities in a meaningful and productive way.
We work to identify the site of pain, decrease inflammation and provide pain relief to help patients improve the quality of their life. Pain that comes from injured or inflamed nerves can often be improved by injecting certain medications to the area of injury. These treatments also allow patients to work effectively with physical therapists and start personal exercise programs to regain strength and function with less pain. Our multidisciplinary chronic pain programs include fibromyalgia services, independent medical evaluations/second opinions, medical acupuncture and industrial rehabilitation.
We offer a variety of appointment times to best fit our patient's busy schedules. Please call to determine the best appointment time for you.
