Cardiac Rehabilitation

A New Outlook on Living with Heart Problems

Northwestern Lake Forest Hospital’s Cardiac Rehabilitation Program is designed to help you achieve and maintain your potential in heart health and functionality by providing integrated, comprehensive services and support. Our heart specialists designed the Cardiac Rehabilitation Program specifically to help you regain your strength, vitality and enjoyment of life.

Whatever your specific condition, the Cardiac Rehabilitation program gives you the opportunity for steady improvement. Our medical director and staff of RNs, respiratory therapists, exercise physiologists, pastoral caregivers and dietitians will develop a coordinated program that includes evaluation, education, nutrition and exercise to gradually increase activity levels and help you reach new lifestyle goals.

We commonly see patients with the following conditions:

  • Angina
  • Angioplasty
  • Coronary artery bypass surgery
  • Heart attack
  • Heart valve surgery
  • Hereditary or lifestyle factors that put you at risk
  • Progressive heart disease
  • Stent placement

The outpatient program includes two phases, and you may enter either Phase II or Phase III with the approval of your physician. Throughout each phase of the program, you’ll be setting goals—and achieving them. And week by week, you’ll be feeling more confident and in charge of your life once again.

Phase II

Phase II is the first level for patients who are new to Cardiac Rehabilitation, and it begins shortly after you leave the hospital. Phase II includes:

  • Exercise three times each week
  • Oversight from healthcare professionals, who are present to monitor heart rate, heart rhythm and blood pressure
  • Lifestyle and risk factor education

Phase III

Phase III is appropriate for patients who have progressed through Phase II and are ready for more vigorous activity. This phase is appropriate for those who simply want to minimize their risks through lifestyle changes, and includes:

  • Individualized exercise plan you follow according to your schedule
  • Group stretching at the beginning of each session
  • Circuit training and exercise of your choice
  • Pulse, heart rhythm and blood pressure checks at regular intervals

Families Are Welcome to Participate

Along with the exercise conditioning, the program teaches you how to make changes to your diet and manage your stress. We find that families of our patients often use this information to improve their own health.

Your family can support new ways of eating at home and help you stick to other lifestyle changes as well. Consider bringing your spouse along; for an additional fee, your spouse can join the Phase III exercise sessions.

We recommend checking with your insurance carrier regarding your coverage. The Northwestern Lake Forest Hospital Cardiopulmonary Rehabilitation Staff will be happy to assist you with insurance questions for either Cardiac Rehabilitation or Pulmonary Rehabilitation.
